Please note that to ensure you receive the correct ADHD screening, the following age guidelines apply:

• ‘Adults’ are aged 18 and over.
• ‘Adolescents’ are aged 13-17.
• ‘Children’ are aged 6-12.

We will request your date of birth at checkout. If this is not provided, we will assume you are an ‘adult’.

Please enter the name and address of the respondent, even if they are a child. The report and letter will be addressed to them.

This ‘supported’ option includes a 1-hour personal online appointment, to support you with understanding and answering the questions.

*The ‘mental health professional’ will be a Psychiatrist, Clinical Psychologist, or Psychotherapist with a minimum of 20 years of experience.

**If you score below the clinical cut-off, your letter will be addressed to you, explaining why you likely do not have ADHD.
